Fixed Braces: Types and How They Work

Fixed braces move teeth using a simple principle: a shaped wire held in brackets tries to return to its own shape, and in doing so it carries the teeth with it. Because the brackets are bonded on, the force is continuous — every hour of every day, with no cooperation required from the patient. That is why braces remain the most versatile orthodontic appliance despite everything newer.

How the force is actually delivered

Each bracket is a handle bonded to a tooth. The archwire threaded through them is manufactured in the shape of an ideal arch. When it is engaged into teeth that are out of line, it is deflected — and the wire pulls the teeth towards its own shape as it straightens.

Wires are changed over the course of treatment. Early wires are thin and highly flexible, able to bend a long way and deliver light continuous force to badly displaced teeth. Later wires are thicker and stiffer, which allows precise control of root position and of the bite. Elastics hooked between the upper and lower arches do most of the bite correction. Small auxiliaries — springs, chains, and occasionally small temporary anchorage screws — are added where specific movements need extra control.

The types

Stainless steel brackets

The standard appliance. Strong, precise, the most economical option, and the one with the longest record of predictable results. Modern brackets are considerably smaller than the ones most adults remember from their own school years.

Ceramic brackets

Tooth-coloured and far less conspicuous at conversational distance. They are the usual choice for adults who want fixed treatment without metal across the front teeth. They are slightly bulkier, more brittle than steel, and the clear elastic ties used with some systems can pick up staining from coffee, tea and strongly coloured food between appointments.

Self-ligating brackets

Instead of an elastic tie holding the wire in place, the bracket has a built-in clip. They are quicker to adjust and hold no elastic ties to discolour, which many patients like. Claims that they shorten treatment substantially are not well supported; treat those with caution. They are available in both metal and ceramic.

Lingual braces

Bonded to the inner surfaces of the teeth, so they are genuinely invisible from the front — the only appliance that is. The trade-offs are real: they take longer to get used to, they affect the tongue and speech more in the early weeks, they are more technically demanding to fit and adjust, and they cost more.

What daily life is like

Discomfort. Teeth feel tender for two to four days after fitting and after each adjustment. Soft food and ordinary pain relief cover it.

Your cheeks and lips. The first week or two involves some rubbing while the soft tissue toughens up. Orthodontic wax over any bracket that is catching solves it, and the problem largely disappears after the initial adjustment period.

Food restrictions, which are not optional. Hard and sticky foods break brackets and distort wires: nuts, hard bread crusts, ice, toffee, chewing gum, and anything you would normally bite into with your front teeth. Cut apples and carrots into pieces rather than biting them. Every breakage is an unscheduled appointment and lost treatment time.

Cleaning, which matters more than usual. Brackets trap plaque, and poor hygiene during fixed treatment can leave permanent white marks on the enamel — a real and entirely avoidable outcome. Interdental brushes, more time, and genuine attention are required. See keeping teeth clean with braces.

Breakages and what to do

A loose bracket is not an emergency but does need rebonding — contact the clinic. A wire poking into the cheek can be eased with wax, or the end tucked down gently with the back of a spoon or clean tweezers, until you can be seen. A wire that has come out of the last bracket should be reported the same way. Never cut a wire yourself unless you have been specifically told how.

Braces or aligners?

Braces handle a wider range of movements, particularly significant rotations, root movement, closing large extraction spaces and substantial bite corrections. They also remove the compliance variable entirely, which is why they remain a common choice for younger patients. Aligners are removable and far less visible. Frequently asked questions

Do ceramic braces stain?

The brackets themselves do not; the clear elastic ties can, and they are changed at every adjustment. Self-ligating ceramic brackets avoid the issue.

Will braces damage my enamel?

Bonding and removal do not, when done properly. Poor cleaning during treatment can, which is why hygiene is emphasised so heavily.

Can I play sport?

Yes. Wear a mouthguard designed for use with braces for contact sports.

Will they affect my speech?

Conventional braces barely, and briefly. Lingual braces noticeably, for a few weeks.

Do braces hurt more than aligners?

They cause more irritation to the lips and cheeks. The pressure sensation when teeth are moved is similar with either.
