How Tooth Decay Develops

Tooth decay is not a hole that appears. It is a slow imbalance between mineral being dissolved out of the enamel and mineral being put back — and in its first stage, before the surface breaks down, it can be reversed.

Understanding it as a process rather than an event changes what you do about it, because it explains why how often you eat sugar matters far more than how much.

The process

1. Plaque forms

A film of bacteria builds up on the tooth surface within hours of cleaning. This is normal and unavoidable; the aim is to disturb it regularly, not to eliminate it.

2. Sugar arrives and acid is produced

Certain bacteria in plaque metabolise sugars and produce acid as a by-product. The pH at the tooth surface drops within minutes.

3. Mineral dissolves out of the enamel

Below a critical pH, calcium and phosphate begin to leave the enamel surface. This is demineralisation.

4. Saliva repairs the damage — if given time

Saliva neutralises the acid, and it carries calcium and phosphate that redeposit into the enamel. Fluoride, if present, makes this repair faster and produces a more acid-resistant surface. This is remineralisation, and it takes perhaps half an hour to an hour to get going after each acid attack.

5. The balance tips

If acid attacks come faster than repair can keep up, mineral is lost overall. The enamel surface becomes porous below an intact outer layer.

6. The surface breaks down

Eventually the weakened surface collapses and a cavity forms. At this point the process is no longer reversible — the tooth cannot rebuild a hole.

7. It progresses inwards

Through enamel into dentine, which is softer and decays faster, and if left, into the pulp. That is when pain arrives and root canal treatment or extraction becomes the question.

Why frequency beats quantity

This is the single most useful thing to take from the process above. Each exposure to sugar starts one acid attack and one recovery period.

A whole chocolate bar eaten at the end of a meal produces one acid attack. The same bar eaten square by square across an afternoon produces a dozen, none of which gets a full recovery period. The total sugar is identical; the damage is not remotely comparable.

The same applies to sipping. A sweet drink finished in five minutes is far less damaging than the same drink carried around and sipped for an hour.

This is why the practical advice is to keep sugar to mealtimes rather than to eliminate it — which is both more effective and far easier to sustain.

The stages you can see

White spot lesions — chalky white marks, usually at the gum line, most visible when the tooth is dried. This is demineralisation with the surface still intact, and it is reversible with fluoride and better plaque control. Most people never know this stage exists.

Brown discolouration — more advanced, often with the surface beginning to break down.

A visible hole or a dark shadow.

Pain — late, not early. A cavity can be well established and entirely painless.

What tips the balance towards decay

  • Frequent sugar and frequent acidic drinks.
  • Plaque left undisturbed, particularly at the gum line and between the teeth.
  • Dry mouth — less saliva means less neutralising and less repair. Caused by many medications, by some medical conditions, and by mouth breathing. See dry mouth and your teeth.
  • Deep grooves in back teeth that a brush cannot enter — see dental sealants.
  • Exposed root surface, which decays more readily than enamel.
  • Existing restorations with worn or broken margins.
  • Not enough fluoride.

What tips it back

  • Brush twice daily with fluoride toothpaste, and spit rather than rinse so the fluoride stays on the teeth.
  • Clean between the teeth daily. A brush cannot reach where teeth touch, which is where a great deal of decay starts — see cleaning between your teeth properly.
  • Keep sugar to mealtimes.
  • Water between meals, not juice, squash or fizzy drinks.
  • Cheese or sugar-free gum after a meal, both of which help neutralise acid and stimulate saliva.
  • Address a dry mouth if you have one.
  • Regular check-ups, so early lesions are found while they are still reversible.

Frequently asked questions

Can a cavity heal itself?

An early white-spot lesion can remineralise. Once the surface has broken down, no — it needs restoring.

Are some people just prone to decay?

Susceptibility varies with saliva, enamel quality and bacterial profile. But diet frequency, plaque and fluoride dominate, and all three are modifiable.

Does sugar-free mean safe for teeth?

For decay, largely yes. For erosion, no — sugar-free fizzy drinks are still acidic and still dissolve enamel.

Why do I get decay when I brush twice a day?

Usually because of what happens between the teeth, or because of how often sugar is reaching them. Brushing alone does not address either.

How fast does decay progress?
